The fact that couching has not traditionally been practiced in the more Southern areas of Africa, such as the Congo, could suggest that couching arrived in conjunction with Islam, and, in any event, probably did not start well South of the Sahara. For instance, if couching had started in the areas of modern-day Nigeria, then it probably would have moved Southward with the waves of Bantu migration in the Common Era.


The era of planned cataract extraction came in the mid-1700s (174). An extremely technically proficient surgeon could pull off cataract extraction with the tools available at that time, but many surgeons continued to prefer couching. Surgeons did not initially use pharmacologic dilation with belladonna, did not have anesthesia, did not understand the need for asepsis, and did not have operating microscopes. For all these reasons, a corneoscleral suture was not feasible. It is astonishing that anyone could successfully perform cataract extraction in the 1700s, given all of these limitations. And yet, there were some successful, high-volume itinerant cataract extractors in Europe, such as Baron Wenzel, Jean-Francois Pellier, and Frederick William Jericho, the latter of whom is the earliest identified oculist to introduce cataract extraction to the New World by 1776 (4,13).

Experience with media usage by the FPA (Family Planning Association of Ceylon between October 1969 and December 1970 is summarized. During this time, the Association purchased 100-200 column inches each of contract advertising space in 26 newspapers. The press has published 268 press release, I.P.P.F., U.N., features and international press clipping in addition to specialized medical articles on family planning methods and 8 articles by FPA office-bearers. In January 1970; the Association launched local radio's first 5-minute daily commercial in Sinhala and Tamil. The program was repeated from April to July 1970. A series of 5 slides on family planning has been shown in movie theathers and more sets are being prepared for viewing. Posters have been used on buses and are currently on display on the National Railways project. Folders, leaflect, and poster calendars have been produced and used. Family Planning stickers have put up in 700 barber saloons. The FPA had stalls in the 1970 3-day National Exhibition at Batticaloa, the 4-day U.N. Poster Exhibition at Badulla, and the 2-week Ceylon Medical College Centenary Exhibition in Colombo. The Information Unit of FPA has answered 18, 541 written inquiries. A family planning communication us regularly dispatched to members of the Cabinet, government and opposition members of parliament, senators, chairmen of local bodies, and key trade union officials.


How does the data collection XIV-XV of the European Society of Human Reproduction and Embryology (ESHRE) PGD Consortium compare with the cumulative data for data collections I-XIII? The 14th and 15th retrospective collection represents valuable data on PGD/PGS cycles, pregnancies and children: the main trend observed is the increased application of array technology at the cost of FISH testing in PGS cycles and in PGD cycles for chromosomal abnormalities. Since 1999, the PGD Consortium has collected, analysed and published 13 previous data sets and an overview of the first 10 years of data collections. Data were collected from each participating centre using a FileMaker Pro database (versions 5-12). Separate predesigned FileMaker Pro files were used for the cycles, pregnancies and baby records. The study documented cycles performed during the calendar years 2011 and 2012 and follow-up of the pregnancies and babies born which resulted from these cycles (until October 2013). Data were submitted by 71 centres (full PGD Consortium members). Records with incomplete or inconsistent data were excluded from the calculations. Corrections, calculations and tables were made by expert co-authors. For data collection XIV-XV, 71 centres reported data for 11 637 cycles with oocyte retrieval (OR), along with details of the follow-up on 2147 pregnancies and 1755 babies born. A total of 1953 cycles to OR were reported for chromosomal abnormalities, 144 cycles to OR for sexing for X-linked diseases, 3445 cycles to OR for monogenic diseases, 6095 cycles to OR for PGS and 38 cycles to OR for social sexing. Sixteen years of medical records documenting 19 births within a herd of Asian elephants ( Elephas maximus) at a private facility in the southeastern United States were reviewed. Of the 19 calves, 11 were normal at birth, requiring no additional veterinary care, and eight were abnormal, requiring veterinary care immediately or within the first week of birth. Descriptive statistics were used to evaluate morphometrics, vital signs, and behavioral milestones in newborn calves both normal and abnormal. Blood work and urinalysis results from all calves were compared to values for adult elephants. Medical management of abnormal calves is described. All calves had faster heart rates and respiratory rates than did adult elephants, but rectal temperatures were the same. Calves were precocious with regard to sitting and standing but could be very slow to nurse. The most-common medical conditions of newborn calves were umbilical abnormalities and problems associated with nursing. Two calves required cardiopulmonary resuscitation after birth but made full recoveries. Some conditions were not apparent at birth but were recognized a few hours or days later. Following veterinary intervention, six of the eight calves made full recoveries, suggesting that early identification and treatment of problems can greatly decrease mortality. This is the first report of multiple veterinary and behavioral parameters in normal and abnormal neonatal Asian elephants from a facility with a calf survival rate above 90%. This information may be helpful to other elephant-holding facilities in providing care to their newborn elephant calves.

Table 1 shows the accumulated maximum operation time (planned uptime), actual hours of operation, and variance (unplanned downtime) for the period October 1-December 31, 2010, for the fixed sites. To determine the accuracy of ultrasound in the diagnosis of congenital abnormalities at the Aga Khan University Hospital, Karachi. The data of congenital abnormalities was obtained from the obstetrical database and medical records of all cases complicated by congenital abnormalities, delivering from January 2001 to December 2003 and was reviewed. Antenatal ultrasounds had been performed by operators with different level of experience. In addition this data was retrieved from the termination and Congenital anomaly register. A structured data collection form was used to collect information of different variables of interest. Congenital abnormalities, complicated 2.8% (n=170), of all deliveries, including all cases of termination of pregnancy, stillbirth and live births. Out of the total, 11.6% occurred in women above the age of 35 years. Consanguinity was found in 18.2% cases. Prenatal diagnosis was made in just under half of the cases (48.8%). Central nervous system and renal abnormalities were commonly diagnosed. However, facial defects, heart defects or skeletal defects were more commonly missed. Antenatal ultrasound successfully diagnosed foetal abnormalities in 48.8% of cases, and more than 90% Central Nervous system defects and renal abnormalities. In contrast about a quarter of Cardiac defects and none of the facial defects were detected. Based on these findings we recommend that the Sonologist should incorporate four chamber view of the heart and also look at the face carefully.


Atlantic basin seasonal hurricane predictions have been issued by the Tropical Meteorology Project at Colorado State University since 1984, with early December forecasts being issued every year since early December 1991. These forecasts have yet to show real-time forecast skill, despite several statistical models that have shown considerable hindcast skill. In an effort to improve both hindcast skill and hopefully real-time forecast skill, a modified forecast scheme has been developed using data from 1950 to 2007. Predictors were selected based upon how much variance was explained over the 1950-1989 subperiod. These predictors were then required to explain similar amounts of variance over a latter subperiod from 1990 to 2007. Similar amounts of skill were demonstrated for each of the three predictors selected over the 1950-1989 period, the 1990-2007 period, and the full 1950-2007 period. In addition, significant correlations between individual predictors and physical features known to affect hurricanes during the following August-October (i.e., tropical Atlantic wind shear and sea level pressure changes, ENSO phase changes) were obtained. This scheme uses a new methodology where hindcasts were obtained using linear regression and then ranked to generate final hindcast values. Fifty-four percent of the variance was explained for seasonal Net Tropical Cyclone (NTC) activity over the 1950-2007 period. These hindcasts show considerable differences in landfalling U.S. tropical cyclones, especially for the Florida Peninsula and East Coast. Seven major hurricanes made Florida Peninsula and East Coast landfall during the top 15 largest NTC hindcasts compared with only two major hurricane landfalls in the bottom 15 smallest NTC hindcasts.
